At Home Care provides personal care services to individuals requiring complex physical support.  We have specilised adult and paediatric teams to ensure that your needs are met to the highest standard, by those who specialise in your specific needs.At Home Care is able to provide a service in this manner due to our ‘nurse-led’ model. This model provides all clients with a Nurse who has clinical oversight of all aspects of their care regime.  This includes:<ul><li>Working with you to develop a Nursing Care Plan, that is tailored to your needs</li><li>Clinical training with your team in your specific care needs, and your Nursing Care Plan</li><li>Providing 24/7 clinical on call support to your team of Support Workers</li><li>Regular follow ups with you and your team to ensure everything is working to your expectations</li></ul>We specialise in working with people living with the following disabilities<ul><li>Spinal Cord Injury</li><li>Acquired Brain Injury</li><li>Motor Neurone Disease</li><li>Huntingtons Disease</li><li>Parkinsons Disease</li><li>Stroke</li><li>Multiple Sclerosis</li><li>Cerebral Palsy</li><li>Other neurological conditions that require complex physical skills</li></ul>We speacialise in supporting people with the following care needs (among others):<ul><li>Bladder and/or bowel care</li><li>Enteral feeding</li><li>Ventilation (both invasive and non invasive)</li><li>Epilepsy management</li><li>Manual Handling / transfers</li><li>Tracheostomy / airway management</li><li>Suctioning </li><li>Cough assist</li></ul>
