At Home Care is able to provide a service in this manner due to our ‘nurse-led’ model. This model provides all clients with a Nurse who has clinical oversight of all aspects of their care regime.  This includes:

• Working with you to develop a Nursing Care Plan, that is tailored to your needs
• Clinical training with your team in your specific care needs, and your Nursing Care Plan
• Providing 24/7 clinical on call support to your team of Support Workers
• Regular follow ups with you and your team to ensure everything is working to your expectations
